PARAMEDIC RELICENSURE – IMPORTANT INFORMATION: November 2, 2017

If your paramedic license expires within six months (180 days), you must take action to renew your paramedic license. In the sixth months prior to the expiration of your paramedic license you should receive a postcard from the California State EMS Authority. If you have moved since your last renewal and did not notify them of your new address, you must obtain a change of address form and renewal application from the State EMS Agency by calling them directly at (916) 323-9875 or contacting them via their website at www.emsa.ca.gov/Paramedic. PROCEDURE:
 Complete your paramedic application. You may attach the Alameda County Fire Department Statement of Continuing Education to your application or transcribe it onto the back of the application. You are required to document 48 hours of CE to re-license. (See additional EMS Bulletin Board post for instructions on how to print CE lists from RMS and Target Solutions.)

 Transcribe any CE you received from other providers onto the back of your application.

 If you are subject to audit, follow the instructions on the letter and provide the additional documents as requested.

 Make a copy of your application and your CE Statement for your records and forward the original completed application packet via department mail to ACFD Admin., Attn: Finance, not less than 60 days prior to the expiration date of your paramedic license. Finance will confirm via email, receiving your application. Finance will obtain funds from the County Auditor and mail the completed application packet with payment to the State EMS Authority. Finance will provide the envelope. Finance will confirm via email that your application has been sent. If you miss this 60 day deadline, Finance will not process your application. You will need to turn in your application on your own.

o The State EMS Authority must receive your renewal application prior to the last 30 days of your license expiring or they will assess a $50.00 late renewal penalty which Alameda County Fire Department will not reimburse you for. If you submit your application to Administration less than 30 days in advance of your license expiration date, include your own personal check for $50.00 Payable to the EMS Personnel Fund with your paperwork.

 On Receipt of your New Paramedic License, complete a Department Form 81.006 Request for Reimbursement, requesting the amount of $500.00 for paramedic license renewal bonus and attach two (2) legible copies of your new license and forward it via department mail to ACFD EMS for approval. Your documentation will be used to update Department records (Telestaff and Fire RMS) and a copy of your license will be kept in your file. Your re-license bonus request will then be forwarded to payroll.
IF YOU MISSED THE DEADLINE AND PAID ON YOUR OWN, SEE INSTRUCTIONS BELOW FOR REQUIRED DOCUMENTS FOR PERSONAL REIMBURSEMENT AND SEND TO ADMIN:

1. Complete claim form 81.006 for $200 reimbursement.

2. Form of payment.
o If paid by check, copy of cancelled check, front and back.
o If paid by credit card in person, original credit card receipt. Copy will not be accepted by the
County Auditor.
o If paid by credit card online or original credit card receipt is lost, email confirmation of
payment, and copy of bank statement. Bank statement must show details of purchase, date, amount,
and be from EMS.
3. Receipt of payment from EMS.
o If paid in person, obtain original receipt and send. Copy will not be accepted by the County
Auditor.
o If paid online, email showing “Order Confirmation” and payment information.
